What ambient replaced
The first generation of digital documentation was dictation: the physician performed the note out loud, during or after the visit, and software transcribed it. The note still cost the physician's words and the physician's time. Ambient tools removed the performance. The visit is a normal conversation, and the note is drafted from it.
What it changes in the room
The screen stops competing with the patient. No typing during the visit, no dictation after it, eye contact for the whole encounter. For the minutes the patient is present, this is a real improvement, and it is why ambient scribes have spread quickly.
What it leaves untouched
An ambient scribe begins working when the conversation begins. The chart review before the visit, the fax queue that refills all day, and the coding after the visit are outside its hearing. The note it drafts also starts from nothing but the conversation: whatever the conversation did not mention, the note does not know.
