AI chart auditability: why source citations matter

An AI note is only useful if the physician can see where each important claim came from and remove what the record does not support.

Polished prose is not proof

AI can write fluent clinical sentences that look finished. That is exactly why auditability matters. The physician needs to know which sentence came from a lab, a prior note, a hospital discharge summary, a patient questionnaire, or an outside consult.
